Highmark Partners with Welltok to Close Gaps in Care and Improve Community Health Among Seniors

DENVER, CO – October 13, 2016


Welltok, the leading consumer health enterprise Software as a Service company, today announced it has partnered with Highmark Inc., the fourth largest Blue Cross and Blue Shield-affiliated company. As part of this collaboration, Highmark’s Medicare Advantage Passport program members will get rewarded for the healthy actions they take towards optimizing their health through the CaféWell Rewards platform.


The Highmark Passport program was designed to help simplify the journey to better health for Medicare Advantage members by creating a single destination for seniors to access information related to their health and wellbeing, including benefit information, savings opportunities and a personal care plan. As part of their ongoing efforts to close gaps in care and make their community healthier, Highmark is leveraging CaféWell Rewards to drive even greater engagement with their senior population. Examples of rewardable behaviors include completing an Annual Wellness Visit and getting an HbA1C screening for diabetic members. Participating seniors will be rewarded for their actions via their choice of gift cards available through the program.
